  1. Do you take insurance? We do not accept insurance. We are committed to providing you an assessment that is detailed, comprehensive, and timely, without constraints placed by insurance panels. Another benefit of a private psychoeducational assessment is that it allows you the freedom to choose the clinician conducting the assessment. At the end of the evaluation, we can provide you a “superbill” for potential partial reimbursement from your insurance (see more information below).
  2. Will my insurance cover a part of the assessment cost? While Mind Map does not bill insurance panels directly, many insurance companies do provide partial reimbursement. At the end of the assessment, we can provide you with a superbill to submit to your insurance. In order to receive a better understanding of your out-of-network benefits, here are some questions you can ask your insurance provider: 
    • What is the reimbursement for the following CPT codes?
      • 90791, 96136, 96137, 96130, 96131
    • Is there a limit on the units of 96137 that I can be reimbursed?
    • Do I need a preauthorization? If so, please request the pre-authorization forms and submit them to us at least one to two weeks prior to your first appointment.
    • How much is my deductible?
  3. How much does an assessment cost? The cost for an assessment depends on the complexity and scope. Please contact us for an estimate of the assessment cost. The hourly rate for consultation is $225 per hour. If you are interested in our services, we provide a free 20 minute consultation to see if we would be a good match for your needs. We provide a “no surprises” good faith estimate for all services in our contracts. Please email info@mindmaptesting.com for more information.
  4. How do I schedule an assessment? Please call us, fill out an information form, or send an email and we will get back to you in a timely manner and schedule the assessment appointments. A $250 nonrefundable deposit is required to hold the appointment dates. The deposit is deducted from the final cost of the assessment, which is due before the report is presented during the feedback meeting.
  5. What makes Mind Map different? Understanding human behavior is complicated. It requires carefully chosen assessment batteries, detailed review or records, gathering collateral information, knowing which questions to ask, and careful interpretation of multiple sources of data. At Mind Map, we don’t cut corners. We delve deeply into learning about your child, their strengths, their vulnerabilities, and provide you with a roadmap that guides you, your child, and the adults that work with them.
